[vtkusers] AMR example

[Marie-Gabrielle Vallet]

I'm looking for an example of AMR. The only one I found is the
HierarchicalBoxPipeline, where a grid hierarchy is read from files. What I
want to do is to create and manage such a structure.

Does anyone has an example ?
