[vtkusers] AMR example

I'm looking for a VTK example of AMR (adaptive mesh refinment). The only one
I found is the HierarchicalBoxPipeline, where a grid hierarchy is read from
files. What I want to do is to create and manage such a structure.

Does anyone have an example ?
