[vtkusers] Blank [White] Dicom images

I recenctly fixed an issue with the vtk DICOM reader. Could you give a 
try to either VTK CVS or VTK-5-0 (from CVS, not tarball).

If this does not work, either try GDCM(*), or send me the image so that 
I can have a look at any potential problem (make sure for instance that 
the image is raw, and not compressed).
