[vtkusers] VTK4.4 + MacOSX 10.3 Darwin + gcc3.1: doesn't build

Has anyone succeeded in building VTK 4.4 on Mac OS X, with the supplied
gcc (which is 3.1)?  It doesn't work for me.  It runs afoul of the fact
that mbstate_t is defined one way in /usr/include/wchar.h and another way
in /usr/include/darwin/3.1/g++-v3/cwchar.  There are #ifdef's in those
files that are supposed to avoid this problem but they don't seem to be
having the desired effect in this case.
