[vtkusers] Missing defines/routines

Lisa,

Bill Lorensen tells me that you have been reworking the vtk volume code.  I am trying to compile some
existing code which makes reference to the define "VTK_FRAMEBUFFER_VOLUME_MAPPER" originally in the
file "vtkVolumeMapper.h" and to the routine "InitializeTextureMapping" originally defined as a member
function of "vtkVolume".  Can you tell me what are the replacements for these?  Thanks.
