Hello David,

The classes used to generate the other two images were never checked in to
VTK - the average ray cast function is trivial to write (just modify the
MIP one as you mentioned). The depth one requires a bit more thought
because it is tricky to set the range of depths. One reason I never checked
these classes in was that I never truly cleaned them up (just wrote them
quickly to generate the images then threw them away). Another reason was
that I wasn't sure if they were generally useful.
