[vtkusers] vtkThreadedStreamingPipeline removed from VTK 6.1.0

We took those out because:
* The classes were largely unused by the community and poorly regression
tested.
* Maintaining them was a hindering other pipeline advancements.
* The author's research direction moved beyond them and was no longer using
them.

$VTKSRC/Examples/ParallelProcessing/SMP is a more recent approach to
threaded pipeline parallelism.
