You should use GetPixelSpacing method.

I render a CT volume using  vtkDICOMImageReader, how can I find out the real
world dimension of a line i.e. if the line end points coordinate are  (
x1,y1,z1) and (x2,y2,z2) I need to know the length in  millimeter of the
line
