[vtkusers] Create an actual mesh from a set of "connected" triangles

I expanded the model by warping the surface in the normal direction, i.e.:
1. generate normals (triangle splitting off)
2. warp (vectors)
The mentioned triangles at the top fin of the dolphin do not "expand" or
"blow up" since they share all three nodes.
