Please look at the examples:

    InsightApplications/Auxiliary/vtk/
        itkReadITKImageSegmentShowVTK.cxx
        itkReadITKImage3DSegmentShowVTK.cxx


You will also find a more sophisticated
application in


    InsightApplications/
         DeformableModelSimplexMesh


This one requires you to have ITK, VTK and FLTK.
