[vtkusers] I can't find some example codes of the VTK Textbook from the CD.

Dear all,
        I got both the VTK User Guide 4.2 and VTK Textbook (3rd ed.).
The CDs (Version 4.2.1) of the above books are the same. However, I 
can't find some examples codes of the VTK Textbook from the CD such 
as comblso.tcl, hawaii.tcl, headSlic.tcl, and ironPlso.tcl etc. 

       Does anyone know where I can get these example codes?
