[Paraview] [EXTERNAL] extract CTH parts (UNCLASSIFIED)

Rick/Alan,

Can you guys try the attached patch to see if the holes issue is
fixed? I am going to further cleanup the filter, but I want to make
sure the holes were indeed caused by this particular bug before I
change more code.
