[Midas] Recent Java updates

Hi Michael,
Unfortunately changes to Java and general browser/OS deprecation of Java have made these problems more and more common. Some ideas:
1) The JAR file that ships with Midas is self-signed. Many browsers have start do reject this outright, but there is generally a way to enable acceptance of these. Another option would be to rebuild the jar yourself and sign it with a key valid for the website upon which you’re running the Midas instance.
2) The standard upload in the latest “master” of the repository supports large files (over 2GB) using newer Javascript APIs. You could achieve the same functionality using the standard uploader, merging the files, and running the DICOM extractor on the resulting item.
