I put in a couple of changes to the ImageFileReader such that if you requested a volume but the
ImageIO object only reads a slice, it will produce a volume with a single slice.  The changes were
fairly local and only required using some default settings for the dimensions that have no data.
 
Steve, can you try these changes with the Meta image IO objects?
