[vtkusers] Layers of vtkImageStack not visible

I am able to more consistently reproduce the problem now.

I am using two monitors on my computer. I've observed that whenever the
executable loads on the right monitor (identified as primary on windows),
the layers are all properly displayed on all three anatomical views.
However, when the application loads on the left monitor (identified as
secondary on windows) the layers beyond the first one do not appear on the
axial view. 

I've also tried unplugging one of the monitors. The problem persists on one
port but not on the other, when only one monitor is plugged in. 

Has anyone observed such an issue before?
