Thanks so much.  That seems to be the issue. I don't know why I couldn't find
that bug report on the Qt forum.  

Based on the comments in the bug and stack overflow
(http://stackoverflow.com/questions/26944831/using-qvtkwidget-and-qopenglwidget-in-the-same-ui)
I will try creating custom QVTKWidget3 which uses QOpenGLWidget instead.
