Thanks very much for your reply, David!  I will take a look at your class.

As a side note, when I read in the same dataset as an ITK image and then use
itkImageToVTKImageFilter to convert it, I find that the ITK and VTK origins
agree.  However, I take it from what you've said that this is only true
because the patient orientation in this patient is the identity matrix.
