Did this ever get answered?  I noticed it's missing as well.  There's a page
for it in the nightly build of the html documentation (
http://www.vtk.org/doc/nightly/html/classvtkSmartVolumeMapper here ). 
Perhaps I should just grab the git repository but I don't want accidentally
run into unstable code.
