[vtkusers] Some d3plot files cannot be read by vtkLSdynaReader

Hi Kevin,

I'll take a look at it over the weekend if no one else can get to it  
first. The reader should never crash, so something is wrong; I don't  
know whether it's related to a missing mesh refinement (d3plotaa) or  
not.
