[vtkusers] QVTKApplication.h

I just ran that example to be sure.  It works, so it seems to be
up-to-date.  The only thing I did was ran cmake and then make,
there were some warnings but no errors.

The header file QVTKApplication.h is in VTK/GUISupport/Qt, but
cmake should automatically configure the include path for you (like
it did for me).

Are you sure that VTK_USE_QT was ON when you built VTK?
