[vtkusers] Create an actual mesh from a set of "connected" triangles

"Clean to grid" creates an unstructured grid. If you use it, you must convert
the model back to polydata with "Extract surface." Or you can use the
vtkCleanPolyData filter with PointMergingOn().

Using paraview, once I do "clean to grid", none of the meshing operations (subdivide, Loop subdivision) are available.
