[vtkusers] VTK + gtkmm + vtkmm (mac osX)

[Justin Giles]

gtkmm is strictly c++.  Making the change to Carbon instead of Cocoa got rid
of the seg fault.

Now I just need to figure out how to get my test image to show up properly.
