Have you looked at this: <http://www.vtk.org/Wiki/Cocoa_VTK> ?

It is a nicely written step by step guide how to build VTK for the Mac. 
I suggest starting with that.  It will give you an idea how to build
VTK, then you can customise the build as you need.
