I started collecting some of my VTK samples at my site www.devsample.org.

There is a sample project called "Volume Render DICOM Dataset " there
on the front page you can take a look at which should render a DICOM
dataset using only VTK code. There is another sample which shows how
to put it into a win32 MDI. Maybe these will help,
