[vtkusers] FEM meshes to VTK converters

An Abaqus input deck reader was started by someone in my group.  He posted
it to the list without really cleaning up the docs, or implementing all the
element types vtk has.  You can find it by searching for
vtkAbaqusInputDeckReader.  You also need the vtkAbaqusElementModel, which I
would like to replace with vtkHierarchicalDataSet, but I have not had the
time.  Also on my plate is an .odb Reader for vtk, using the Abaqus
libraries, which I have not started.  If you have any questions about the
vtkAbaqusInputDeckReader I'll do my best to answer them.
