You should use vtkOrientationMarkerWidget, documented here: 
http://public.kitware.com/dashboard.php?name=vtk.  Also, there has been 
discussion of this on this mailing list before; please search the mailing 
list archives (http://www.kitware.com/searchpublic.html) before posting 
questions to the list.
