[vtkusers] 'operator <<' is STILL ambiguous

This error occurs when you build VTK 4.2 with VTK_USE_64BIT_IDS set to on
with MSVC 6.  It has been fixed in VTK 4.4 and the CVS version.  I've just
merged the fix to the 4.2 branch.  Checkout VTK with "-r release-4-2" to
get it.
