If it is urgent you can go there:
http://public.kitware.com/cgi-bin/cvsweb.cgi/VTK/Examples/Medical/Cxx/?cvsroot=VTK

and download the example you want (it has been updated to run with 
VTK4). Otherwise you can wait for VTK4.2 to come out...which should be 
imminent.
