Hi Will,
it seems to me, that you have used a very old version of the
perl-script, which translates the old documentation style to doxygen
syntax, and that you have used an old version of doxygen and an old
version of the configuration file for doxygen (the doxyfile).

you can find all new information on:
ftp://sic.sp2mi.univ-poitiers.fr/pub/barre/vtk/doc/

the documentation looks much nicer now,
* examples are referenced
* index is sorted by the 4th character
* search engine!
* ...
