vtk 3.1 Win32 problems

Hi David,

Are you sure the examples that you are running came from the VTK 3.1 
distribution? volSimple.tcl and vol.tcl no longer have SetScalarInput (it 
changed to SetInput a couple of revisions ago....). That makes me 
suspicious of the other problems as well.
