Windows 2000 and VTK 312

I cut a new patch release of VTK 31 last week titled VTK 312.  The main 
reason for this patch was due to some incompatibilities between windows 
2000 and VTK's Tkwidgets.  While I was at it I threw in some other recent 
fixes as well. Mainly.

1) ExtractVOI and ExtractGrid not working correctly
2) Connecting a StructuredPoints directly to an ImageViewer now works
3) TIFFReader not working on 64bit systems.
