[Ves] Updated support for latest iOS and Android versions - and VTK upgrade

Hi folks,

I merged a branch into 'next' called 'upgrade-toolchains-and-vtk'.  This
branch updates the android toolchain file to support Android NDK v8b.  It
also updates the iOS toolchain and KiwiViewer Xcode project file to support
Xcode 4.5 and iOS 6.0.

The branch also updates the sha1 of VTK to a version based on VTK master
from today.  Previously, the kiwi library was built against a version of
VTK from May.

After some testing on additional machines, I hope this branch will be OK to
merge to master later this week.  If anyone decides to try out the branch,
please let me know your experience.  I think an incremental build should
work OK, but since it is pulling a new version of VTK, a clean build would
certainly not hurt.

I have tested the updates using:

OSX Mountain Lion
Xcode 4.5, iOS 6.0 simulator and iPad 2 with iOS 6.0
Android NDK v8b and api android-16; using device Samsung Galaxy Nexus with
Android 4.1

Ubuntu 12.04
Android NDK v8b and api android-16; using device Samsung Galaxy Nexus with
Android 4.1

I think that earlier versions of the Android NDK, Xcode, and iOS should
still be work as before, but I no longer have earlier versions installed my
development machines.
