[Paraview] PVD file format

This question comes up every once in a while.
So, I just ported Appendix D from the old Paraview Guide to the wiki (and
updated the timestep description to be float instead of counting number.)

I _think_ the answer to both of your questions is no but don't have time to
verify.
Try it and see.
