[Paraview] Grid is shifted in different dimensions

Andy,

Please find attached my data.
I suspect there might be something wrong with my way of writing VTK 
files. Can you confirm this file is ok or not?
It is desirable to avoid my own filter since I'm not familiar with 
python and ParaView filters.
