[Paraview] PV 3.8.0 mesh modification

I have a mesh, I would like to refine it in a region (for example the 
wake behind a cylinder or a sphere).

I can refine the mesh using PV (sub divide).
Questions:
1- How can I choose a region where I want to subdivide?
2- Once subdivided, how do I save the new mesh?
3- What formats are available?
