[Paraview] Enabling a vtk filter example missing xml

I am trying to follow the "Enabling a filter in VTK"
(http://www.itk.org/Wiki/Plugin_HowTo#Enabling_a_filter_in_VTK) guide
on the wiki. This example is how to make a paraview plugin for an
existing VTK filter. It looks like the CMake file is provided, but the
real details are left to the book (I'm assuming it's the book - the
link is broken). Does anyone have the server manager xml and gui xml
that go along with this example? That seems like something that
definitely needs to accompany this example on the wiki.
