[Paraview] bug when adding new readers in plugins

I've found a problem in the latest CVS version of Paraview.  The example
program ParaView3/Examples/Plugins/Reader does not add the extra file type
when trying to open a file.

Another problem I have found is that the reader from the plugin appears in
the "Sources" menu (this was the same before I updated to the latest CVS
version).  Is there a way to stop this as it depends on the filename.
