Hi Sam,


You should look at:

1) ITK Software Guide
    http://www.itk.org/ItkSoftwareGuide.pdf

    In particular the chapter "Reading and Writing Images".


2) The source code in

    a) InsightApplications/LiverTumorSegmentation
    b) InsightApplications/DigitalArchive
