[Insight-users] Papers and Authors

All,
  This is not an ITK related question but figured that there are a lot 
of Ph.D.'s in academics on this mailing list to answer this question :
     Besides the whole "quid-pro-quo" thing (you include me as an author 
in your paper, i will do the same) what is the etiquette and protocol 
for including someone's name on the author list ? Is intellectual 
contribution the only factor ? What about cases when someone wrote the 
code that was used to produce results reported in the paper ? Or helping 
someone with conducting the experiments (but not processing the data) 
for a paper ? How do you guys decide who to include on the author list ?
