Hello Jared.
I've changed the example of QT + ITK.  Now it compiles and executes with 
no problem.  Here I send you a zip file with the code and if you want 
any explanation of the changes just tell me.  I'm not an expert on 
either ITK or Qt, but currently I'm working with the toolkits.

Here the details of what I'm using:
Qt: 4.1.4. open source version
ITK: 2.8.1 - release
CMake 2.4 patch 2.
Compiler: Visual C++ 2005 Express Edition
