[Insight-developers] Gerrit changes prior to modularization

I've now reviewed all open changes in Gerrit.  I abandoned those that
will not merge cleanly with an explanation inline.  Others I tested
to verify that they will merge cleanly and added a review note.

Please base all new work on the latest 'master' as usual.
