[Insight-developers] Renaming txx to hxx and the Wiki

The summary if the reason for this is detailed in the commit message, and
in the issue trackers that describe them.  I'll also make a copy of it for
the migration guide.

Leila,

Apparently, many IDE's (like Visual Studio, Eclipse, ...) do not
recognize .txx as a valid c++ file and must be specially configured.

We used .txx in the early days because VNL used it. Apparently it is
an extension the VNL folks invented.

We discussed which extension to use and .hxx was the most acceptable.

It should have 0 affect on users since they never should be including
the .txx file.
