[Insight-developers] Mngw Build needs attention

The build amber12.kitware--Win32-mingw-gcc-4.5 has not been compileing
for a few days. It looks like a configuration problem to me, I have a
personal mingw build that is fine.

Something to do with threads. My build uses
CMAKE_USE_PTHREADS:BOOL=OFF
CMAKE_USE_WIN32_THREADS:BOOL=ON

Looks like the subject build has both PTHREADS and WIN32_THREADS ON.
