The BRAINS2 I/O issues are (hopefully) related to the BRAINSMaskIO readers.
We've stopped using that format, and I really hope no-one else uses it
because it is not suitable for representing physical space.

We had intended to remove that a long time ago, but I forgot about it.  I've
just made a pass through all of BRAINS packages and removed it.

I've also just made the necessary changes to Slicer4 to remove references to
the Brains2Mask format.  I've also ran a little internal script

%~/src/brains2/variable_conversions.sh itk::OStringStream std::ostringstream

That does global file search and replace in all source code.

Preliminary build completed without a problem.  All tests requiring GUI
failed due to shared library issues.  I'll need to reconcile those before I
submit.
