on the users list that the SetOutputParametersFromImage and
SetOutputParametersFromConstImage methods from
Review/itkOptResampleImageFilter.h do not copy the start index of the
input image as is done in the SetOutputParametersFromImage method of
BasicFilters/itkResampleImageFilter.txx.
