[Insight-developers] InsightApplication QtITK for qt4

hi all,

I modified slightly the insight application code to work with QT4.
it's been quick and nasty, but it works.
I put a copy here if someone is feeling motivated/bored enough to  
merge it in Insight application for the example to work with any  
version of Qt.
