[Insight-developers] .net 2003

I've checked in a bunch of changes so that it should now build without
errors on .NET2003.=20

The major changes were:
* The logic for ITK_TYPENAME was flawed
* added a bunch of typenames to itk_hashtable
* .NET2003 had some issues with argument of type foo[Dimension]
 In particular it does not like foo[TImage::ImageDimension],
 or foo[itkGetStaticConstMacro(Dimension)]
 or when foo is declared foo[] in *.h but foo[Dimension] in *.txx


The changes seems compatible with bcc, VC60 and VC70 (see Sunday's
experimentals).

I did also check that it builds on Cygwin-gcc3.2 - the experimental
build (no errors) and the tests ran - but it couldn't submit - it fails
somewhere in the tcl ftp class.
