[Insight-developers] Tests reworked

I found it, it is the old #include <fstream> first in any .cxx file that
may include <fstream> indirectly.  Luis and I searched the ITK archives and
found the solution.   

The good news is that this bug is not in Visual Studio 7.

I have checked the fix in Testing/Code/IO (I added #include <fstream> 
as the first include for VTK, META and Raw IO test.cxx files.)
