[Insight-developers] All image iterators now support Adaptors

I check in all my changes to the ImageIterators to support adaptors.  This means:

1. ImageIterators are templated over image type instead of pixel type and dimension.
2. operator* has been removed in favor of Set()/Get()
3. ImageIterators have a method Value() that bypasses the DataAccessor code.

I change all the code that uses ImageRegionIterator to the new API.  Changing some of the algorithm
code was tricky, so there may be a couple of outstanding run-time issues that I missed. I also had to
change some of the DataAccessor code to ferret a few reference and const issues.

Now that all the image iterators have a uniform API, I'll start looking at simplifying the iterator
dereferencing code.
