How about the main categories being:

build process - any problems with building or installing Insight
documentation - incorrect, missing documentation
source code - errors or missing features in the source code
testing - failed tests, inadequate tests, memory leaks


validation - bugs in validation, I'm not sure how these are different from 
the above four categories. Does anyone have a comment on this?

I lumped all the classes into source code.  I'm not sure we need to 
differentiate between the functionality.  I do think that we could add a 
category for third party software such as the numerics package we use etc. 
I'm not sure if that should be one category per package or one general 
category for third party packages.

Any other comments?
